“Serious failures” in ******** military practices were responsible for the deaths of *********** aid worker Lalzawmi ‘Zomi’ Frankcom and six of her colleagues, according to a report commissioned by the *********** government.

The report found the ******** Defence Forces (IDF) did not follow correct procedures, pointing to “mistaken identification and errors in decision-making,” Foreign Minister Penny Wong said in a statement on Friday.
