RT while in Sword mode and Perfect Guard by hitting the input at the moment your foe's attack is about to hit you. Unlike a regular guard, a Perfect Guard will not consume weapon sharpness and negate all damage.

The Power Clashes, on the other hand, can only be triggered by Perfect Guarding against certain attacks multiple times. Given that consistency, I don't recommend focusing on triggering the sequence at all. Instead, treat it as a welcome bonus for a solid defense, and don't take unnecessary risks.
